I'll try to reproduce this.* I've tried inciting revolts against various enemies, using spies and diplomats. When I try to enter the city an options pop-up comes up list invite revolt, investigate city, etc. Click on incite revolt closes the pop-up, and opens another with the same options. This can be repeated indefinitely.
It's in Combat menu, and key shortcut is Shift+F (so plain F always, no matter the unit, stands for Fortify)* The build fortification options for workers / engineers seems to be missing. I tried hitting "F", and pulling down the Unit and Work menu. No luck.
